Dogs with diagnosed chronic kidney disease (CKD) should have their diet managed in close partnership with a veterinarian. The appropriate level of phosphorus restriction, sodium management and protein depends on the CKD stage (IRIS 1–4). How the Kidneys Work and Why They Fail
The kidneys are filtration organs that remove metabolic waste products from the blood, regulate electrolyte balance (sodium, potassium, phosphorus), control hydration, maintain blood pressure and produce hormones including erythropoietin (which signals red blood cell production). Each kidney contains around 400,000 tiny filtering units called nephrons.
Chronic kidney disease (CKD) occurs when nephrons are progressively destroyed — by age, genetic factors, infection, toxin exposure, immune-mediated injury or other causes. Because the kidneys have enormous reserve capacity, clinical signs of CKD typically don't appear until 75% or more of nephron function has already been lost. By the time a dog is showing symptoms — increased thirst, more urination, reduced appetite, weight loss, vomiting — meaningful kidney damage has already accumulated.
This is not a reason to despair. Well-managed CKD can be stable for years. Diet is central to that management.
The Four Dietary Pillars of CKD Management
Decades of research in veterinary nephrology have established four core dietary priorities for CKD dogs. They are not equally weighted — phosphorus restriction and hydration are the most critical — but all four matter.
1. Phosphorus Restriction (Most Critical)
Damaged kidneys cannot excrete phosphorus normally. As phosphorus accumulates in the blood (hyperphosphataemia), two damaging things happen. First, the body tries to correct the calcium:phosphorus ratio by dissolving calcium from bones — mediated by parathyroid hormone. This secondary hyperparathyroidism causes bone demineralisation and deposits calcium salts in soft tissues. Second, high serum phosphorus is directly toxic to the remaining functional kidney tubule cells, accelerating the destruction of nephrons you are trying to protect.
Phosphorus restriction is the single dietary intervention with the most consistent evidence for slowing CKD progression in dogs. The goal is to keep serum phosphorus within normal limits through dietary management, and sometimes with phosphate binders (medications that bind phosphorus in the gut before it absorbs).
2. High Moisture — Hydration Support
Dogs with CKD frequently develop a concentrating defect — the damaged kidneys cannot concentrate urine properly, leading to increased urination and the risk of dehydration. Dehydration further stresses the kidneys and can precipitate acute kidney injury on top of chronic disease. High-moisture foods — fresh food, wet food, broth added to meals — directly support hydration in a way that dry kibble fundamentally cannot. This alone is a strong argument for homemade or wet food formats in any CKD dog's diet.
3. Protein: Quality Over Quantity
Protein metabolism produces nitrogenous waste products (urea, creatinine) that impaired kidneys struggle to excrete. Historically, veterinary nutrition for CKD focused heavily on protein restriction. Current thinking is more nuanced. Excessive protein restriction, particularly in early CKD, may cause muscle wasting without meaningful benefit. The current evidence suggests that protein quality matters as much as quantity — highly digestible, biologically complete proteins from animal sources produce fewer waste products than lower-quality proteins for the same amino acid yield. For early to moderate CKD, keep protein adequate and high-quality. In late-stage CKD (IRIS Stage 3–4), protein restriction under vet guidance may be appropriate.
4. Omega-3 Anti-Inflammatory Support
EPA and DHA from salmon oil and oily fish reduce inflammatory processes in the kidney that accelerate nephron destruction. Multiple studies in dogs with CKD have demonstrated that omega-3 supplementation slows the rate of kidney function decline. Always add salmon oil cold to cooled food — heating destroys its benefit.

Foods to Avoid for CKD Dogs
Organ meats — liver, kidney, heart — are among the highest-phosphorus foods available and should be strictly avoided in CKD dogs. This is a significant departure from general homemade diet advice, where organ meat is encouraged for its micronutrient density. In CKD, the phosphorus burden outweighs those benefits.
Sardines with bones are the usual recommendation for omega-3, but whole sardines with their bone content are high in phosphorus. For CKD dogs, sardine oil or boneless salmon fillets are better omega-3 sources. Dairy products including cheese, yoghurt and milk are high in phosphorus and should be avoided. High-sodium processed treats should also be eliminated — sodium management matters in CKD because impaired kidneys cannot regulate sodium and blood pressure as efficiently.
💧 Water Intake: Make It Easy
Beyond food moisture, encouraging water intake matters for CKD dogs. Multiple fresh water bowls in accessible locations. Some dogs prefer running water — a pet fountain can meaningfully increase intake. Adding a small amount of low-sodium broth to the water bowl can attract reluctant drinkers. For CKD dogs, water intake is as important as diet composition.
Breeds at Higher CKD Risk
Several breeds have documented predispositions to CKD. Cocker Spaniels, Bull Terriers, Cairn Terriers, Samoyeds and German Shepherds are among those with higher rates of hereditary nephropathies. Smaller breeds generally have higher rates of age-related CKD than large breeds. Dalmatians are prone to urate stone formation due to altered purine metabolism, requiring a different dietary modification than CKD management per se. For any predisposed breed, annual bloodwork and urinalysis screening from middle age is worthwhile.
